>> Cincinnati police are investigating what went wrong, after a teenage boy was crushed to death by the rear seat of his Honda minivan, as he pleaded for help from 911 operators. 16 year old Kyle Plush had become trapped, when the seat flipped backwards as he reached for his tennis racket.

>> One thing that we do know, is that on that second 911 call, something has gone terribly wrong. This young man was crying out for help. We weren't able to get that information to the officers on the scene and we need to find out why. I'm not certain at this point, if we're talking about an equipment malfunction or some type of other user error possibly, but we're gonna do an investigation to get those answers.

>> Police say they checked the parking lot of Seven Hills School but found nothing wrong, and dismissed the call as a prank. Kyle's father found the boy's body in the van about six hours later. Before the 911 call disconnected, officials say the boy told them, tell my mum that I love her if I die.
